Beneficiary Forms
Though it is often an overlooked topic, estate planning is a key factor in retirement planning. As we enter retirement, we all must think about and plan for the future of our families by ensuring our affairs are in order. Many of us completed our beneficiary forms years ago when we were first employed and have not thought much about them since then. Do you know who is listed on your beneficiary forms for your unpaid compensation, life insurance, annuity, or Thrift Savings Plan? Have you divorced, remarried or had children since the last review of your beneficiary forms? These are questions you should consider when planning your estate. If you are unsure of the answer to these questions, we recommend that you review your eOPF to ensure that all of your beneficiary forms are up to date.
Generally, the last forms on file in your e-OPF will determine your beneficiaries. If there are no designated beneficiaries on file, benefits will be paid in the following order of precedence:
- Widow or widower.
- If none, child or children in equal shares, with the share of any deceased child distributed among that child's descendants.
- If none, parents in equal shares or the entire amount to your surviving parent.
- If none, the executor or administrator of the estate.
- If none, next of kin who would be entitled to the estate under the laws of the state in which you resided at the time of your death.
You do not need to make a designation if you are satisfied with the order of precedence that the law provides. Click the links below to access the fillable designation of beneficiary forms.

Federal Employees Retirement System (FERS) Forms
- (SF-3107), Application for Immediate Retirement (FERS)
- SF-3107-2, Spouse’s Consent to Survivor Election This form is only required if you do not elect the full survivor benefit for your current spouse. It should be included with the (SF-3107)
- (SF-2818), Continuation of Life Insurance Coverage
- TSP Forms needed for your election of TSP disbursements
- SF-3108, Application to Make Service Credit Payment (FERS)
- SF-3112, Documentation in Support of Disability Retirement Application
Civil Service Retirement System (CSRS) Forms
- SF-2801 Application for Immediate Retirement (Civil Service Retirement System)
- SF-2801-2, Spouse’s Consent to Survivor Election; This form is only required if you do not elect the full survivor benefit for your current spouse. Include with the SF-2801 form.
- SF-2818, Continuation of Life Insurance
- TSP Forms Needed for Your Election of TSP Disbursements
- SF-2803, Application to Make Deposit or Redeposit (CSRS)
